Patient Information and Admission
Your doctor or dentist will arrange admission to Western Day Surgery. You will be given a Patient Registration form to complete and send to us. If you do not receive this form please phone us on (03) 9366 0666 to have one posted to you or print it from the Forms section of this website.
This Patient Registration form needs to be received by our Admission Officer at least 48 hrs before the day of your procedure as we need to advise you of the cost of the procedure prior to your admission date. Please ensure that if you have a mobile phone that your number is included on the Patient Registration form.
Fax your completed Patient Registration form to (03) 9366 0777 or post or deliver it to the Western Day Surgery at 145 Furlong Road, St Albans Vic 3021. If you post the form, remember that we need to receive it 48 hours before the day of your procedure.
On the day before your procedure you will be contacted by our Admissions Officer and advised of your admission time and any particular pre-operative information.
Important Information You Need to Know
- Stop smoking at least 24 hours before surgery
- If you are taking medication, continue to take it with a small sip of water only
- Do not eat or drink for 6 hours prior to surgery. Surgery will be cancelled if you do not comply
